Mission vs. profit

As organizations scale, incentives drift from the person served to the shareholder. Sometimes what's best for the company is not what's best for the customer.

When a company scales and goes public, something quietly shifts. The people it answers to change—from the patient in the chair to the shareholder on the call. Sometimes what's best for the company is just not what's best for the customer.

I see it clearest in my own field. The bigger these dialysis giants get, the harder they are to turn, and the easier it is to lose track of who they were built to serve. Corruption works like gravity—a slow pull toward least effort. That's the thing to watch.
